Cohabiting Partner Nomination Form (SR83 Issue 6 Sep 2022)

Since April 2014 you are no longer required to nominate a cohabiting partner to be eligible for a survivor pension.

However making a nomination helps us establish eligibility in the event of your death.

If you are married or are in a registered civil partnership, do not complete this form. Your husband, wife or registered civil partner is automatically covered for a survivor pension.
