Use this form to request investigation of previous pension rights from non-Local Government Pension Schemes including Personal Pension Plans (complete a separate form for each Pension Scheme you want us to investigate).

Once we have received this form we will request details from your previous pension scheme and come back to you with a quotation and an Option Form to complete.
